MCHCP is pleased to offer Lifestyle Ladder, the wellness program provided through Gordian Health Solutions, for 2008.
What is Lifestyle Ladder?
Lifestyle Ladder is a program designed to improve health and educate employees of the forces that can impact their health and quality of life. When health risks are identified and managed appropriately, the result is a healthier, happier more productive individual who has taken a proactive approach to lower individual and overall health care costs.
Am I eligible to participate?
If you are a current non-Medicare subscriber and eligible to enroll for coverage under MCHCP's medical plan prior to October 1, 2007, you are eligible to participate in Lifestyle Ladder.
Why should I participate?
- Participants of Lifestyle Ladder learn how to manage health risks in order to maintain a higher quality of life.
- Participants of Lifestyle Ladder can receive up to $24 monthly in medical incentives. That's a $288 annual savings in medical costs!
How do I participate?
To participate in the Lifestyle Ladder program, a Personal Health Analysis (PHA) must be completed annually during MCHCP's Open Enrollment (October 1-31, 2007). Eligible members will have access to the online PHA through myMCHCP. Paper PHAs will not be available.
Participation includes taking scheduled calls from your health coach.
